I have found that Shimano shoes have more spacious toe boxes than most and are extremely comfortable and are sensibly priced. R088s will take 3bolt sl or 2bolt spd cleats and are reasonably stiff for racing and general use. Likewise Shimano RT82s are good all rounders though not as stiff and are spd 2 bolt only with a sleek rubber sole for walking in.
There is an adapter SH SM 40 available to fit smooth soled shoes ( R088 eg ) for spd cleats. I have these on my R088s combined with Shimano A600 pedals and they are great !
Shimano wide fit xc shoes...
Bont MTB shoes too are quite wide and can be had in a wide fit.
